I have applied for a lease and hereby authorize you to release to K-Property Mgmt. Inc. the requested
information in this form concerning:
1. Employment history, dates, title, income, etc.
2. Rental verification (begin date, end date, rent amount, payment record etc.)
3. Any information deemed necessary in connection with a consumer credit report for real estate lease
transaction.
4. If I, the applicant tender a security deposit for an apartment, that particular apartment will be
removed from K-Property Management’s Available Apartment List. It is understood that if I, the
applicant am approved and do not execute a Lease for that apartment , I will forfeit my security deposit.
5.My application fee of $50. will be applied as a credit for rent if my application is approved and my
lease term is at least 12 months. This fee is not refundable if I am not approved.
The information on this form is for the confidential use of the lessor in determining my credit worthiness
for a lease and/or to confirm information I have supplied. In addition, I am aware that the
documentation supplied is subject to re-verification after the date of the lease execution.
A photographic or FAX copy of this authorization may be deemed to be the equivalent of the original and
may be used as a duplicate original. The original signed form is maintained in the office of K-Property
Mgmt. Inc.
